Choose the tryout date and time slot for your player's grade. Players are encouraged to attend both tryouts — please submit a separate registration for each. The $15 tryout fee applies to players trying out with CPBA for the first time and can be paid online after submitting, or by Zelle to pay@coachpoole.com. Returning CPBA players try out free.

Participation Waiver & Release of Liability *I understand that basketball participation with Coach Poole Basketball Academy involves inherent risks including but not limited to sprains, fractures, concussions and, in rare cases, serious injury. On behalf of myself and my player, I assume those risks and release the Academy, its coaches, staff, volunteers and facility partners from claims arising out of participation, except for gross negligence or willful misconduct.Emergency Medical Treatment Authorization *I certify my player is physically fit to participate. In the event of injury or illness I authorize Academy staff to obtain emergency medical care for my player, and I accept financial responsibility for that care.Academy Policies & Code of Conduct *My player and family agree to treat coaches, teammates, opponents, officials and facilities with respect and to follow all Academy policies, including attendance and fee policies. The Academy may remove a participant for conduct that endangers or disrupts others, without refund.Photo, Video & Media Consent *I grant the Academy permission to use photographs and video of my player taken during Academy activities for promotional purposes, including the website and social media, without compensation.Tryout Fee & No-Refund Policy *I understand that the tryout fee reserves a spot for my player at the tryout session I selected. I understand that tryout fees are non-refundable once registration is submitted, except if Coach Poole Basketball Academy cancels the tryout or as otherwise required by law.
