Participation Waiver & Release of Liability *I understand that basketball participation with Coach Poole Basketball Academy involves inherent risks including but not limited to sprains, fractures, concussions and, in rare cases, serious injury. On behalf of myself and my player, I assume those risks and release the Academy, its coaches, staff, volunteers and facility partners from claims arising out of participation, except for gross negligence or willful misconduct.Emergency Medical Treatment Authorization *I certify my player is physically fit to participate. In the event of injury or illness I authorize Academy staff to obtain emergency medical care for my player, and I accept financial responsibility for that care.Academy Policies & Code of Conduct *My player and family agree to treat coaches, teammates, opponents, officials and facilities with respect and to follow all Academy policies, including attendance, uniform and fee policies. The Academy may remove a participant for conduct that endangers or disrupts others, without refund.Photo, Video & Media Consent *I grant the Academy permission to use photographs and video of my player taken during Academy activities for promotional purposes, including the website and social media, without compensation.Player Fee & No-Refund Policy *I understand that by accepting a roster spot with Coach Poole Basketball Academy (CPBA), CPBA is reserving a limited team position for my player and may decline roster opportunities to other players based on this commitment. I understand that player fees are non-refundable once a roster spot is accepted and registration is submitted, except if CPBA cancels the team or as otherwise required by law.
